it`s called a pity smash the player that`s losing the most gets it it depends on the situation of when u get it though

uhh no when you die 5 times withought getting one kill you automatically get the smash ball... i dont belive you can turn this off... im not sure

You still had the final smash turned on, but u put the item frequency to none, if you do this there is a sort of karma system that is enabled and if you are losing you sometimes get a final smash

i was actually a bit surprised when i saw this video, this happens all the time when you turn the smash ball off.
